We at the CAC are pleased to help you with your social service and medical transportation needs. To begin, please fill out the form below. In addition, you need to send us a copy of your State Photo Identification (license or ID card), a copy of your birth certificate, a copy of your social security card, verification of your income, and a copy of your Ohio Medicaid Card (if you have one). Should you have no income, you will be asked to sign a “No Income” form on the day of your transport.
Transportation is available for appointments with the Belmont County Department of Job and Family Services, any government agency, social service and medical appointments: doctor’s office, hospitals, clinics in Belmont County, and neighboring counties in Ohio and West Virginia.
A 2-week notice is required for all CAC transportation.

We are a private organization, founded in 1965, administering many federal, state and locally funded programs. These programs target areas such as housing, utility assistance, education, health, nutrition, youth employment programs, and transportation. It is our goal to alleviate the conditions of poverty in our county.
